The HospiceCare mission is to provide quality of life at the end of life for terminally ill patients and their families. Hospice is a unique kind of care that provides comfort and support for people with life-limiting illnesses and their families. Expert pain and symptom management and compassionate counseling allow patients to live each day to the fullest, in their homes or in a homelike setting, such as nursing homes, facilities offering assisted-living service or hospice inpatient centers. Hospice is an option for people with a life-limiting illness with a life expectancy of less than six months. The patient must make a conscious decision to seek palliative or comfort care over curative care. Referrals to hospice can be made by patients, physicians, family members, friends, neighbors, co-workers, spiritual care advisors or health professionals with a simple telephone call.

One of the major goals is to ensure that all students come to school "ready to learn". Nursing services include assessing and screening students with various health problems including dental health. Nurses also assist students and their families in obtaining appropriate health care services. Nurses work to remove the barriers that may interfere with students receiving health care. These barriers may be lack of health insurance and/or inability to speak English. For example, a student may be come to the health office with a complaint of dental pain. The nurse learns that this student does not have dental insurance and the family is Spanish speaking. The nurse will locate a dentist willing to see uninsured children and make sure their is a Spanish speaking interpreter available. In providing these case management services, the nurse has removed the barriers that may contribute to this student doing poorly in school and advanced their well-being and academic success. In the Madison office of the MDA, we serve 19 counties in SW Wisconsin. We serve over 443 client families in this region. WE assist these families with the purchase of medical equipment such as wheelchairs, leg braces, voice computers, therapy, physician visits, etc. We also assist the client families by offering them support through clinics and support groups that are held monthly. We are able to offer our client families so much support due to our fundraising efforts. We run several fundraising events all year long. We do fill-the-boot events with community fire departments, lock-ups, the Jerry Lewis Labor Day Telethon, Hope-A-Thons, Harley-Davidson rides, Shamrocks, etc. We need all of the help that we can get in order to fulfill our mission of helping our client families to the fullest extent of our abilities. Established in March of 1991, Neighborhood Connections serves between 10-20 people with various forms of support specific to the needs and desires of the individual being served. We offer three programs: The Parent Stressline provides a listening ear and resource referrals to parents and caregivers 16-hours a day, every day. The Families United Network (F.U.N.) provides parent and youth support groups and the Oasis program offers group treatment for children, adults and families affected by sexual abuse. They learn when it is appropriate to report problems to Case Managers so that referrals to other services can be made. Elder Friends are paired with clients in need by Case Managers. They are asked to visit their clients weekly for at least six months to a year. The progress of the relationship is monitored by Case Managers and by the Peer Support Coordinator. By implementing these services, we prevent blindness by identifying early signs of vision disorders, facilitating early and effective treatment,and preventing eye injury. We are a not-for-profit organization and are supported entirely by voluntary contributions from individuals, corporations, family foundations, organizations, special events and bequests. We are not a United Way agency and we do not receive state or federal funding. Prevent Blindness makes a difference through free screenings for very young children through older adults, educational programs, eye health and safety materials, presentations and workshops. Prevent Child Abuse Wisconsin Prevent Child Abuse Wisconsin is the state chapter of the national child abuse prevention organization, Prevent Child Abuse America. PCAW does not provide direct service, but does provide information to concerned citizens and professionals through the 1-800-CHILDREN information phone line, handouts, pamphlets, and our website. PCAW provides opportunities for involvement and advocacy for survivors of abuse through the Banner Project and the Art of Healing Display. PCAW hosts the Annual Together for Children Conference, partners with media, and provides materials to local prevention groups to assist them in their community outreach efforts. PCAW advocates for policies that have a positive impact on children and families through communications with national and state representatives and by providing information and strategies to people across Wisconsin to support them in advocating for issues related to child abuse prevention. United Cerebral Palsy) is a vital nonprofit agency providing direct services to over 1,000 individuals each year. 90% of agency funds are directed to local program services, with only 3% spent on administrative and fundraising costs. UCP provides services to individuals with developmental disabilities or delays such as autism, epilepsy, cerebral palsy, Down syndrome or delays in speech, mobility or development. HealthCheck (HC) is Wisconsin’s Early and Periodic Screening Diagnosis and Treatment Program (EPSDT). It is a comprehensive preventive screening of children through the age of 20 who are on Medicaid. Legislation mandates that HMO’s must achieve an 80% screening rate. In order to meet the 80% mandate, Unity has developed several HealthCheck (HC) programs to increase member awareness and compliance. Preventive healthcare services are a priority for Unity. Volunteers will help by making phone calls in order to get the children for their screening. In fact, 90% of the 800,000 people with CFS nationwide are on disability. The illness is systemic, affecting every organ system in the body. With the name chronic fatigue syndrome, comes the misunderstanding that the illness is about feeling very tired. In fact, cognitive problems, tremendous weakness and continual viral flu like symptoms cause an inability to sustain work activity. Patients can do little to alter the name which was set by federal health agencies in 1988 and altered from the original name, Myalgic Encephalomyelitis. SO, securing medical and social services help is difficult. We need healthy volunteers to work with patients and to work with the Association as we press forward. The Wisconsin Women's Health Foundation is the home base for many different programs throughout the state of Wisconsin, our programs focus on the greatest threats to women's health, those being: cardiovascular disease, cancer, mental illness, domestic abuse, osteoporosis, and tobacco use. The foundation strives to make a difference in the quality of health care and education for women in Wisconsin. We are seeking men and women of all ages who would be interested in helping with the work of one our programs called "First Breath". Our "First Breath" program is a smoking cessation program for preganant women. We provide health care providers with the information and materials they would need to give smoking cessation counseling and assistance to women receiving prenatal care looking to quit for the health of their baby. The majority of the women that enroll in the problem are low income Medicaid/Badgercare recipients that have many stresses in their daily lives, to keep them motivated to quit smoking we provide incentive gifts for them throughout their pregnancy. Our program is looking to have hundreds of these gifts made before our major training event in July. We are looking for volunteers to make baby gifts which include the tasks of gift wrapping.
